Medical Release - By submitting this form, you are giving the VBS staff permission to administer basic first aid in the event of an injury. Staff will contact emergency services in the event of a significant injury and you will be responsible for such emergency services.

Photo Release - By submitting this form, you grant the church permission to copyright and use photographs/videos taken at VBS in any manner or form for any purpose lawful at any time. You waive your right to inspect or approve the finished product or written copy in conjunction therewith, or the use to which it may be applied.
